Business Flood Damage Assessment Form

All property owners in Jackson County are urged to report flood damage to their homes and businesses as a result of the historic rains at your earliest convenience.

This will help Jackson County assess the total damage to private property and will help show the need for FEMA’s Individual Assistance program to be available.
On June 25th, the Jackson County Board of Commissioners declared Jackson County to be in a State of Emergency due to the flooding throughout the county. That cleared the way for Jackson County to be included in the President's National Declaration for MN. Most typically, aid for a disaster comes in the form of rebuilding government infrastructure. However, when disasters rise to a National Declaration, that can open up relief to homeowners and business owners if a demonstrated need occurs. Hence, there is a need to complete this individual flood damage report to document how widespread the damages are.
You can use the QR code below to submit your information yourself. The QR code was supplied by the Homeland Security and Emergency Management Division of the MN Dept. of Public Safety and will be a collection point for the state as FEMA begins its processes for assistance. The code will bring you to a simple 4-question form with a place to submit pictures if you have some outside damage to yards and landscapes that is not a coverable expense.

Water, sewer backups, and water-related damage as a result of the rain and floods is important to report.
ONLY ONE TYPE OF REPORT IS NEEDED PER HOUSEHOLD. IF YOU HAVE ALREADY PROVIDED A PRINTED OR EMAILED FORM, YOU DO NOT NEED TO COMPLETE THE ONLINE VERSION.
